Osteíte Esternoclavicular e Psoríase em Paciente com Síndrome Sapho: Relato de Caso/Sternoclavicular Osteitis and Psoriasis in a Patient with Sapho Syndrome: Case Report

Abstract
Introdução: A síndrome SAPHO é uma condição relativamente rara, pouco diagnosticada e com características específicas que originaram seu epônimo: Sinovite, Acne, Pustulose, Hiperostose e Osteíte. Casuística: Esse relato de caso apresenta um paciente 37 anos, engenheiro, morador de uma cidade no Sul de Minas Gerais que apresentava dor torácica persistente há 5 anos que não respondia ao uso de drogas convencionais. Discussão: O diagnóstico da Síndrome SAPHO é realizado pela combinação de manifestações clínicas, achados laboratoriais e exames de imagem, com destaque importante para a cintilografia óssea e tomografia computadorizada do esterno. A síndrome SAPHO deve ser incluída nos diagnósticos diferenciais diante de um paciente com lesões escleróticas líticas ou ósseas hiperostóticas. Conclusão: A síndrome SAPHO é doença rara e seu diagnóstico se faz pela combinação de manifestações cutâneas, achados radiológicos e histopatológicos que necessitam de conhecimento adequado pelos médicos para seu tratamento.

ABSTRACT:
Introduction: The SAPHO syndrome is a relatively rare condition, underdiagnosed and with specific characteristics that led his eponym: synovitis, acne, pustulosis, hyperostosis and osteitis. Case report: This case report presents a patient 37 years old, engineer, resident of a town in southern Minas Gerais who had persistent chest pain 5 years ago that did not respond to conventional drugs. Discussion: The diagnosis of SAPHO syndrome is accomplished by combining clinical, laboratory and imaging tests, especially important for bone scintigraphy and computed tomography of the sternum. SAPHO syndrome should be included in the differential diagnosis when a patient with lytic or sclerotic bone lesions hiperostóticas. Conclusion: The SAPHO syndrome is a rare disease and its diagnosis is made by combining cutaneous manifestations, radiological and histopathological findings that require adequate knowledge by doctors for their treatment.
